Case Manager - The Lodge, Time Limited Role

Conduct intensive case management for approximately 20 high-acuity clients, assessing barriers to housing placement and implementing interventions to support housing retention and community integration. Collaborate with clients to develop independent living skills such as budgeting and meal preparation, while coordinating appointments, transportation, and follow-up services for primary and mental health care. Complete comprehensive biopsychosocial assessments, quarterly goal plans, and maintain accurate progress notes and physical charts in compliance with HIPAA and funding requirements. Directly transport clients in your personal vehicle to agencies and healthcare providers across the greater Los Angeles area, while working cooperatively with the multidisciplinary team and property managers.
