Case Manager - The Layover

Job Summary

Conduct participant-centered case management using Housing First, harm reduction, and trauma-informed care approaches to support individuals experiencing homelessness. Perform initial and ongoing needs assessments, develop individualized housing plans, and meet with participants monthly to assist with document readiness and permanent housing transitions. Coordinate resources, facilitate intakes, and provide crisis intervention while maintaining compliant HMIS documentation and participant files. Transport clients in a personal vehicle to appointments and engage in case conferencing with community partners.

Required Qualifications

  • High School Diploma or GED or Equivalent
  • Valid CA Driver's license

  • Willingness to work in a high tolerance Harm Reduction model
  • Experience working with participants with mental illness
  • Experience working with participants with chronic health issues
  • Experience working with participants with substance abuse disorders
  • Experience with Outreach and engagement

  • Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds
  • May be exposed to elements like cold, heat, dust, noise, and odor
  • May need to bend, stoop, twist and sit throughout the day

Desired Qualifications

  • Bachelor's degree
  • a minimum two (2) years' experience working with homeless individuals
  • preferably those living with mental illness and/or substance addictions
  • an equivalent combination of education and experience
  • Minimum of one (1) year experience in case management
  • Knowledge of Housing First
  • harm reduction
  • trauma-informed care principles
  • Experience using HMIS or similar database
  • Strong documentation and organizational skills
  • Ability to work effectively with diverse and high-acuity populations
