Case Manager

Coordinate, monitor, and document case management and service planning for youth in the residential program while facilitating the interdisciplinary treatment team process. Complete admission activities, service planning documentation, visitation planning, and contact privileges within required timeframes. Monitor implementation and progress toward individualized goals, ensuring meaningful youth and family participation in permanency and discharge planning. Maintain communication with families, courts, schools, and community providers, and initiate discharge and transition planning at admission. Represent the agency at judicial hearings, IEP meetings, and other case-related meetings as assigned. Escalate clinical, psychiatric, medical, or significant safety concerns to qualified personnel. Maintain complete, accurate, confidential documentation in the Agency electronic health record and participate in supervision, case consultation, and quality-improvement activities. Participate in the on-call rotation for one week at a time, rotating every seven weeks amongst the clinical team.
