Job Summary
Meet with administration, medical, and nursing staff to plan services for patients and families. Partner with care teams, social agencies, and community resources to ensure smooth discharge transitions. Assist with admission and discharge processes while attending care plan meetings to provide professional guidance on patient needs. Ensure all documentation is informative and adheres to center policies. Maintain confidentiality and follow HIPAA compliance requirements. Make independent decisions and lead harmoniously with other personnel.

- Must possess good judgement
- Must have the ability to maintain highly confidential information
- Must be knowledgeable of HIPAA compliance and requirements
- Must be able to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to the Company
- Must be able to pass a pre-employment background check/drug screen
- Active Fingerprint card
Desired Qualifications
- A minimum one year case management experience (preferred)
