Job Summary
Manage a caseload of up to 30 participants by assessing their strengths and challenges to facilitate individual care plans. Link clients to community resources and provide warm handoffs while assisting in securing necessary documentation and services. Implement Individual Service Plans through home visits, problem-solving, and advocacy at court, school, and Wellness Recovery Action Plan meetings. Conduct administrative functions including heavy documentation, ensuring client file completion and submission. Attend monthly Guam Homeless Coalition Continuum of Care meetings and trainings. Maintain a valid driver's license and vehicle insurance for all duties.
Required Qualifications
- Graduation from an accredited four-year college with a degree in Social Work, Psychology, Public Health, or a related field
- Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance
- CPR Certification and First Aid Certification
Desired Qualifications
- Two (2) years of experience in providing case management
- Intake and assessment experience
- Marketing and/or sales experience
- Bilingual
