Case Manager

Case Manager for Teller County DHS in Woodland Park, CO, responsible for determining initial and continuing eligibility for food stamps, Medicaid, and other public assistance. Responsibilities include gathering and reviewing information from applicants, verifying data via interviews and home visits, informing applicants of program requirements and appeals rights, assisting with completing forms and verifications, approving/denying applications, daily data entry into computer systems, and making referrals to community services. Requires strong customer service, excellent written and verbal communication, ability to interpret complex government rules, and the ability to manage multiple programs. Must have high school diploma or equivalent, 3+ years of public-facing experience, and hold a valid Colorado driver’s license with ability to pass background check and drug screen. Work schedule is 40 hours per week, on-site in Woodland Park, with comprehensive benefits.
