Job Summary
Case Manager RN in home health leading patient-centered care planning. Develops, delivers, revises, and evaluates individualized care plans under physician supervision; provides case management for assigned patients and coordinating staff and services; ensures documentation and data submission (e.g., OASIS, PAR) within required timelines; conducts initial evaluations and ongoing reassessments; advocates for patient welfare and coordinates care among patients, families, agency staff, and outside providers; administers medications per policy, educates on home health strategies, participates in care conferences, and supports nursing staff through supervision, teaching, and on-call coverage; maintains confidentiality, performs QA/QI activities, and communicates changes in patient condition to the physician and team; requires licensure in state(s), ability to meet health requirements, and strong written/verbal communication skills.
Required Qualifications
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
- Minimum (1) year nursing experience required; prior home health experience preferred
- Ability to exercise initiative and independent judgment
- Excellent verbal and written communication
- Must have the physical abilities to perform the described duties
- Able to organize work procedures through time management, assume responsibility and effectively supervise ancillary disciplines per state requirements
- Acceptance of philosophy and goals of this Agency
- Ability to secure transportation and travel as required
