Job Summary
Case Manager RN provides and oversees in-home patient care per physician orders and plan of care. Develops, delivers, revises, and evaluates individualized patient-centered care plans; conducts initial evaluations and regular reassessments; coordinates care among patients, families, agency staff, and external providers; ensures accurate documentation (including OASIS/PAR) and timely reporting; administers medications per orders; participates in clinical rounds and on-call duties; mentors and supervises aides; maintains confidentiality and adheres to infection control and QA plans; requires RN licensure and minimum one year of nursing/home health experience; strong communication, organization, and advocacy skills; CPR certification and ongoing continuing education.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) practicing
- Minimum of 1 year nursing experience; home health experience preferred
- Ability to organize work procedures and manage time
- Performs writing, dictation or keyboard communication of directives
- CPR certification and licensure maintenance
- HIPAA compliance and confidentiality
- Ability to secure transportation/travel as required
- Demonstrates problem-solving and communication skills
- Ability to educate patients/families on home health strategies
- Follows physician orders and documents nursing activities per policy
