Case Manager

Case Manager is a registered nurse responsible for developing, delivering/implementing, revising, and evaluating individualized patient-centered care plans under physician supervision. Will provide case management to an assigned group of patients and ancillary staff, perform initial evaluations and regular reassessments of nursing needs, and coordinate care among patients, families, the agency, and other providers. Duties include documenting via required data sets (OASIS, PAR, etc.), medication reconciliation, education for patients and families on home health strategies, participating in clinical conferences, and ensuring compliance with policies, infection control, and quality improvement programs. Responsibilities also include on-call duties nights, weekends, and holidays; communicating changes in patient condition to physicians and staff; supervising and teaching other nursing personnel; and facilitating referrals to Physical Therapy, SLP, OT, and Medical Social Work as needed. Requires strong communication, time management, ability to secure transportation, and maintaining licensure/continuing education. Salary range: 70,000–85,000/year.
