Job Summary
Interview and evaluate tenants to formulate independent living plans, then locate and provide access to appropriate community resources. Assist individuals in attaining goals by making referrals to services both within and outside CAMBA, while monitoring progress via regularly scheduled telephone contact and face-to-face visits. Document all interactions via progress notes and participate in case conferences. Review documentation establishing tenants' eligibility for programs and make file copies. May escort tenants to appointments and prepare marketing materials for the program. Flexible hours may be needed occasionally for shift coverage until 8 pm.
Required Qualifications
- Bachelor's degree (e. g., B.A., B.S.W.)
- experience working with the seriously and Persistently Mentally Ill (SPMI) population
- equivalent experience to a Bachelor's degree
- Must be fingerprinted
- obtain Criminal History Checks (CHC) with Justice Center for the Protection of Vulnerable People with Special Needs
- Ability to maintain fingerprint clearance throughout the duration of employment
- Mandated Reporter status
- Ability to immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1-855-373-2122
- Ability to complete a CAMBA Incident Report
- Availability for flexible hours occasionally to provide shift coverage until 8 pm
