Job Summary
Conduct intake with new clients within 24 hours of admission, document psychosocial and housing assessments, and develop service plans. Provide comprehensive case management by connecting clients with medical, mental health, and community services while monitoring behavior, medication compliance, and progress toward permanent housing. Facilitate counseling sessions, workshops, and recreational activities, perform room inspections, and assist with building evacuations during emergencies. Maintain accurate client records in accordance with DHS, OTDA, and SUS standards while holding current CPR and First Aid certification.
Required Qualifications
- HS Diploma or GED
- 1-2+ years' experience with homeless population
- Ability to work with a culturally diverse population
Desired Qualifications
- Bachelor's Degree is Human Services field preferred
- Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred
- Bilingual Spanish speaking strongly preferred
- Previous case management experience in a homeless shelter setting
- Experience using the AWARDS application
- Spanish, Creole, or French language skills are a plus
