Case Manager | Supportive Housing Services

Provide psychosocial rehabilitative services as described by the Medicaid manual to support clients in meeting individualized goals and care plans. Complete documentation on services provided within Valley's standards and meet productivity expectations as assigned by supervisor. Distribute medications and transport clients to appointments as necessary while ensuring regulatory compliance and organizational standards are met. Schedule is Monday through Friday, 8:00am to 5:00pm. Compensation depends on certification status, with uncertified staff starting at $17.50 and certified staff at $18.75, with pay increases based on tenure and experience.
