Job Summary
Provide outreach services to people experiencing homelessness, perform case management, and link clients to housing within a mission-driven mental health program. Duties include developing individualized service plans, coaching skills toward goals, coordinating transportation to appointments, creating weekly activity schedules, responding to incidents per policies, engaging clients in community activities, communicating with collaborating agencies, documenting in the client’s EMR within 48 hours, ensuring safety and housing entry for shelter residents, advocating for housing and benefits, conducting monthly house meetings and outreach, and completing routine documentation and program tasks. Skills include direct client support, care coordination, documentation accuracy, collaboration with external agencies, and strong professionalism and compassion.
