Case Manager, Supportive Housing

Provide stabilization case management including assessment, service plan development, and referrals to community-based resources, advocacy, and assistance in obtaining benefits. Conduct brief vocational assessment for the purposes of vocational planning and job placement. Participate as a member of interdisciplinary team of care providers and facilitate the exchange of information for housing related issues affecting consumers. Provide intensive 1:1 case management to clients placed into housing to assist them in maintaining housing, addressing holistic needs (mental health, substance abuse, budgeting, tenancy, life skills, socialization, community supports) to prevent homelessness. Make appropriate referrals to outside resources, act as liaison and advocate with landlords and providers, maintain accurate case records, provide reports, and recruit referrals to the MST program. Conduct outreach and participate in engagement team meetings, support job search activities (application/resume/interview) for clients and liaise with employers as needed. Ensure confidentiality and compliance with policies. Travel across the Metrowest area and participate in program-wide activities as assigned.
