CASE MANAGER SUPERVISOR

Case Manager Supervisor in Santa Fe, NM leads and develops SFRC’s Case Managers, sets best practices and standards of care, facilitates team meetings, provides training for new hires, and interviews candidates. Works with SFRC’s Case Manager Supervisor in Gallup and Santa Fe’s women’s and children’s program to coordinate and improve case management systems organization-wide, ensuring optimal use of community resources, service delivery, and compliance with external agencies and referral sources; facilitates wraparound case management aligned with CCBHC criteria and prepares clients for successful transitions back to the community.
