Job Summary
Case Manager to help individuals experiencing homelessness secure housing, benefits, and community resources. Responsibilities include completing client assessments and orientations, developing individualized case plans, connecting clients to housing and community resources, assisting with benefits, identification documents, and healthcare access, entering accurate HMIS documentation, monitoring progress and adjusting plans as needed, collaborating with internal teams and partner agencies, and transporting clients to appointments. Preferred bilingual English/Spanish. Requires associate degree or equivalent human services experience, valid Arizona driver's license, ability to obtain fingerprint clearance, and strong documentation/computer skills.
Required Qualifications
- Associate degree in a related field, or equivalent human services experience
- Knowledge of community resources
- Valid Arizona driver’s license and clean driving record
- Ability to obtain an Arizona Level One Fingerprint Clearance Card
- Strong documentation and computer skills
