Job Summary
Case Manager for the Safe Harbour Program serving minor victims of human trafficking and youth at-risk in Suffolk County, NY. Responsibilities include conducting risk assessments, providing trauma-informed, victim-centered case management and outreach, developing individualized service plans, coordinating with partner agencies, transporting clients to court and medical appointments, assisting with public benefits and referrals, maintaining case records in the CAC NYSCA database, preparing reports, and participating in trainings to support program implementation. Requires two years of relevant experience, proficiency with computer applications (Google Drive, MS Office, calendar/email or similar), and a valid NYS driver’s license with access to automobile; strong communication and organizational skills are essential.
