Case Manager - Street Outreach

Job Summary
Provide housing-focused services to individuals experiencing unsheltered homelessness, maintaining a caseload of 12-15 participants. Engage participants to progress toward stable housing through housing assessments, coordination with housing programs, and removal of barriers using creative problem-solving. Conduct field outreach (including sites like campsites) and on-site drop-in hours to offer resource navigation and material support. Document via HMIS and ClientTrack, complete weekly reporting, and respond to communications promptly. Work in pairs with team members or partners, utilize a personal vehicle for outreach, and adhere to mandated reporting. Embrace values of anti-racist culture, trauma-informed care, and harm reduction; collaborate with internal teams and external organizations; willingness to travel and work in varying conditions; and preference for candidates with lived homelessness experience and bilingual capabilities. This role operates in Minneapolis, MN and involves travel within the community as part of outreach responsibilities.
Required Qualifications
- Hold MN Driver's License; pass background check; be able to drive company vehicle
- Maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence
- Willingness to lift up to 50 pounds occasionally and walk up to 5 miles
- Experience with Microsoft Office products
- Ability to work outdoors and indoors in extreme weather
- Strong documentation and data-entry skills (HMIS and ClientTrack experience preferred)
- Ability to establish rapport with participants in crisis using trauma-informed, person-centered, strengths-based approaches
- Commitment to harm reduction and Housing First philosophies
- Ability to collaborate with diverse stakeholders (participants, coworkers, partners)
- Two years experience preferred in homelessness services or related field
- Lived experience with homelessness preferred
- Bilingual skills (Spanish, Oromo, Somali, Amharic) preferred
- Community travel within the region; valid driver’s license and reliable transportation required
