Job Summary
Case Manager in Stratford provides comprehensive, ongoing case management and service coordination for residents in First Step House permanent supportive housing. The role supports individuals experiencing serious mental illness, substance use disorders, and homelessness, focusing on housing stability, recovery, and community integration. Responsibilities include building supportive relationships; assisting clients in identifying goals and service needs; coordinating care and community resources; supporting engagement in treatment and recovery services; facilitating life skills and psychoeducational groups; providing transportation and appointment coordination; collaborating with multidisciplinary clinical and housing teams; delivering services onsite and in residents' homes using trauma-informed, recovery-oriented, and strengths-based approaches; and maintaining documentation within HMIS and KIPU.
