Job Summary
Manage a caseload of clients with complex needs, providing ongoing case management, supportive counselling, and crisis response. Complete assessments and service planning in collaboration with clients to help them achieve goals through individualized service plans. Facilitate groups such as life skills and harm reduction, while supporting access to housing, income supports, healthcare, and community resources. Maintain accurate documentation on EMHware and build relationships with community partners to advocate for clients within systems. Assist clients in accessing core services including showers, laundry, meals, and recreation areas, and provide front desk support for registration and referrals. Administer first aid or call emergency services as required. Work collaboratively with staff during the week to provide wrap-around care for elderly homeless men at Haven Toronto's drop-in centre.
Required Qualifications
- Bachelor's Degree in Social Work (BSW) from an accredited institution
- Registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Demonstrated experience in case management, assessment, and working with individuals experiencing mental health and/or substance use challenges
Desired Qualifications
- Training in motivational interviewing and group facilitation
- Additional language skills
