Job Summary
Case Manager (Specialist) provides intensive case management to eligible clients, meeting participants weekly; acts as primary point of contact to coordinate services and care; handles a caseload, conducts comprehensive screenings/assessments across functional domains, develops and implements case plans, coordinates care, advocates before agencies, facilitates group education, and documents sessions. On-site responsibilities require reporting to work on-site daily. Qualifications include an Associates Degree in Social Work or related field (or equivalent experience) and at least 1 year of general case management experience; completion of VOALA Case Management training within 6 months of hire; preferred Counseling Certification; HMIS navigation; E-Verify compliance; participation in mandated reporting. Required Qualifications
