Case Manager - Special Child Health Services

Conduct comprehensive assessments with children and families to identify needs across developmental, educational, medical, behavioral, and emotional domains. Develop, implement, and regularly update Individual Service Plans (ISPs) based on each child's needs, strengths, and family goals. Connect families with appropriate medical, developmental, educational, behavioral health, social service, and community resources while making referrals and assisting families in navigating complex service systems. Serve as a responsive resource for families experiencing challenges, helping address complaints and barriers to care through effective communication and problem-solving. Promote effective communication and coordination among the child's care team, including primary care providers, specialists, educators, therapists, and other professionals. Monitor progress toward established goals and evaluate the effectiveness of services to make recommendations for changes when needs or circumstances evolve.
