Job Summary
Assess and develop individualized service plans to implement services supporting adults with serious mental illness toward recovery goals. Partner with members to foster self-sufficiency, focusing on increased employment, community activity, and independent living. Provide supportive services including monitoring behavioral health services, interacting with families and other parties, and identifying necessary resources. Maintain current CPR, First Aid, and Therapeutic Options certifications while holding a valid Arizona driver's license. Requires a bachelor's or master's degree in behavioral health, excellent communication skills, and proficiency in Microsoft Office.
Required Qualifications
- Master's or bachelor's degree in a behavioral health or related field
- Bachelor's degree in a non-related field PLUS one-year related experience
- Associate degree in any field PLUS one-year related experience
- High school diploma or equivalency certificate PLUS three years' related experience
- Current CPR, First Aid, and Therapeutic Options certifications
- Valid Arizona driver's license
- Car insurance
- At least 21 years of age
- Ability to maintain excellent member and family relationships
