Job Summary

Conduct initial intakes with clients and families, ensuring timely completion of paperwork, assessments, and individual service plans per Southwest Key Programs policy. Complete daily documentation and enter data into the SWK database while maintaining both physical and web-based client files. Facilitate group counseling sessions, provide crisis intervention, and perform weekly home visits with thorough documentation. Transport clients to activities, court appointments, and treatment meetings as needed, and administer emergency First Aid, CPR, AED, and Handle with Care techniques at any time. Maintain open communication with the team and attend required professional training. This role requires a flexible schedule with on-call availability, valid driver's license, and ability to lift up to 20 pounds.

Required Qualifications

  • Bachelor's degree in a social service or related field
  • 2 years' experience working with youth at risk youth and their families
  • master's degree in social services or related field
  • Ability to clearly read, write, and communicate information to internal and external stakeholders
  • Computer proficiency and the ability to effectively operate standard office equipment
  • Works well independently and as an effective team member
  • Clear background check from appropriate entity
  • First Aid, CPR, AED certification
  • Handle with Care certification
  • Valid driver's license
  • Status as a qualified driver under the Company's Vehicle Safety Guidebook standards
  • Reliable transportation with required registration and vehicle insurance
