Job Summary
Conduct intake screenings and complete annual behavioral health assessments, including service planning and support planning. Facilitate Care Team Meetings and ARTs, while coordinating care for members inside and outside the agency. Identify gaps in care and assist members with community resources. Complete SMI and SED evaluations and manage referrals. Work primarily with adults within an interdisciplinary team to deliver compassionate care. Requires a Bachelor's degree in behavioral health with relevant experience, a valid AZ driver's license, and fingerprint clearance.
Required Qualifications
- Minimum of 22-year of age
- Bachelor's degree in the behavioral health field
- one year of relevant experience in behavioral health
- A combination of experience and education totaling 2 years is acceptable
- Valid AZ driver's license
- motor vehicle clearance
- fingerprint clearance issued by the Arizona Department of Public Safety
- be eligible to receive fingerprint clearance upon hire
- SB&H is a drug-free workplace
