Case Manager

Provide in-home, community, or office-based case management and direct services within the Metro Outpatient team, including crisis stabilization, assessments, and development of service plans to support the consumer and family in achieving the least restrictive setting and improved functioning. Responsibilities include delivering 1:1 or group interventions, coordinating with the consumer’s care team, monitoring treatment response, and handling documentation to ensure appropriate community resources and daily living skills are available for adults and their families; travel to home or community sites is required as needed. Minimum qualifications include a Bachelor’s degree with behavioral health experience, a valid Arizona driver’s license, fingerprint clearance, First Aid/CPR certification, and auto insurance; salary and benefits are described in the organization’s documentation.
