Case Manager Social Worker LCSW-Casual Days

Casual part-time Case Manager/Social Worker with an LCSW-C at Northwestern Medicine Lake Forest Hospital. Responsibilities include developing individualized treatment plans with patients and families, coordinating and leading care conferences for patients with longer stays, documenting activities, arranging appropriate discharge planning and transfers to suitable levels of care, and utilizing community resources to support continuing care. The role involves patient advocacy, collaboration with physicians and healthcare teams, monitoring compliance with managed care contracts, and delivering education to patients and families on advance directives. Requires a Master’s degree in social work, licensure as an LCSW in Illinois, and 3–5 years of hospital experience.
