Case Manager Social Worker (LBSW) - Grace Medical Center

Coordinate post-acute care services and provide psychosocial support to patients and families by gathering pertinent information to determine care plans. Develop psychosocial databases, assessments, and intervention plans while participating in interdisciplinary efforts to modify post-acute needs. Provide crisis intervention and counseling services, document assessments and outcomes per departmental guidelines, and actively organize discharge rounds on assigned units. Maintain timely communications with third-party payors to identify discharge needs and educate the treating team on bed utilization and length of stay. Lead departmental operations, improvement projects, and staff development activities while engaging in peer review and staff work groups.
