Job Summary
Case Manager/Social Worker providing Support/Service Coordination for adults with Intellectual Disabilities, Autism Spectrum Disorder, and Physical Disabilities. Develops Person-Centered Service Plans (ISP), coordinates and brokers services, monitors outcomes, and ensures comprehensive, cost-effective care. Duties include assessments, plan development/modification, ensuring provider choice, aligning Behavioral and Crisis Plans with the ISP, reporting abuse/neglect, coordinating with other care entities, researching alternative solutions, maintaining confidentiality, attending trainings, and traveling to participants as needed (approximately 50%).
Required Qualifications
- BS/BA degree in related course work
- Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
- Experience with Microsoft Office, state-maintained databases.
- CPR/First Aid certification and updated state related Clearances.
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration
