Job Summary
Plan and coordinate care activities for patients, including developing discharge plans, scheduling appointments, and communicating with family members and community providers. Formulate and conduct educational, therapeutic, and support groups while participating in multidisciplinary team meetings for patient care planning. Document findings and actions on medical records according to departmental guidelines and support facility-wide quality improvement goals. Provide basic consultation with the treatment team to ensure continuity of care post-hospitalization. Monday through Friday schedule; full-time role with overtime flexibility.
Required Qualifications
- Bachelor's Degree from an accredited college or university in Social Work or a clinical-related mental health field
- Minimum of one (1) year of experience in related disciplines with psychiatric hospital experience
- Crisis prevention/management training within 30 days of employment before assisting in a restraining procedure
- Full-time employment availability
