Job Summary
Monitor and coordinate necessary evaluations, surveys, and assessments to identify individual needs, then develop and distribute Interdisciplinary Service Plans for approval. Assist residents in managing psychiatric symptoms, developing coping skills, and maximizing independence through training in personal and community resources. Provide crisis intervention and stabilization services during scheduled hours and respond to after-hours emergencies per on-call procedures. Accompany individuals to appointments, assist with daily living skills, and increase social support networks while ensuring compliance with occupancy rules. Maintain relevant documentation, statistical reports, and attendance at staff meetings, participating in Resident Council meetings as requested.
Required Qualifications
- Associates Degree in a Human Service field
- A car
- A valid Ohio driver's license
- Insurability through Agency's insurance carrier
- Ability to work a flexible work schedule including evenings, weekends, and holidays
Desired Qualifications
- Prior experience in case management working with individuals with severe and persistent mental illnesses
