Job Summary
Provide holistic case management to support housing stability for participants, including assisting with tenancy education, addressing barriers (e.g., documents, evictions, criminal records), coordinating resources and transportation, maintaining timely documentation in ClientTrack, and building partnerships with property managers. Responsibilities include caseload management (min. 18 participants), conducting intakes, weekly participant meetings with monthly home visits, helping participants access benefits (Housing Support, MFIP, SSI, health coverage), and facilitating connections to education, training, or employment. Emphasizes equity and antiracist practice, professional collaboration, and strong communication to advocate for participants’ housing stability and financial independence; requires a Bachelor’s degree and experience in mental/chemical health issues, plus a valid driver’s license and access to a vehicle.
Required Qualifications
- Bachelor’s degree in human services or equal experience in the field
- Experience with mental health and chemical health issues
- Driver’s license and car required
