Job Summary
Case Manager (QMHS) at Signature Health responsible for development and execution of client treatment plans, coordinating transportation to access resources, collaborating with treatment teams, and supporting both adult and child clients (SMD/SED, CD/Dual DX) across educational, medical, vocational, housing, and daily living needs. The role involves maintaining HIPAA-compliant documentation, facilitating crisis intervention and community outreach, and participating in quality assurance activities. Requires a High School Diploma or equivalent, a valid Ohio driver’s license with insurance, and BLS certification; a Bachelor’s degree is preferred and 1–3+ years of related experience is preferred. Work location is Beachwood, OH (in-person).
Required Qualifications
- High School Diploma or equivalent
- Valid unencumbered Ohio driver’s license and proof of driver’s insurance
- American Heart Association Basic Life Support (BLS) certification
- Bachelor’s Degree in related social services field preferred
- 1-3+ years of previous case management or relevant experience preferred
- CDCA preferred
- Strong computer skills with Microsoft, Excel, etc.
- Knowledge of ASAM levels of care, ability to assist client in obtaining residential, detox, and supported housing services in the community
