Job Summary
Conduct prescreening to determine program eligibility, gather verification documentation, and complete Intakes, Assessments, Service Plans, Reassessments, and Accompaniment. Connect clients to medical, mental health, substance use, legal, and entitlement services while monitoring progress toward housing goals via monthly home visits and scheduled contacts. Escort clients to appointments and assist with benefit applications, ensuring adherence to primary HIV medical care and medication treatment. Enter all client services into eSHARE and maintain audit-ready charts. Provide monthly program reporting data and meet required projections.
Required Qualifications
- Bachelors Degree
- Associates degree

- GED
- at least 4 years' experience in case management or related social service field

- Two (2) years of relevant experience
- mid-sized (250+ employees) organization
- Good written & verbal communication
- Computer literacy in Microsoft Office Suite
- Bilingual in Spanish
- Minimum Education/Experience Required
