Job Summary
Conduct personalized case management for families in the shelter program, guiding them through intake, assessments, and tailored service plans. Document services in real-time, maintain electronic case records, and prepare monthly compliance reports. Connect participants with community resources for housing, employment, and support in areas like mental health, domestic violence recovery, and family reunification. Design workshops to empower life skills and crisis intervention strategies while collaborating with community partners to ensure seamless support. Uphold program policies, ensure regulatory compliance, and attend hearings as needed. Submit insightful reports tracking program impact and participate in team meetings to strengthen our mission of ending family homelessness in the Bay Area.
Required Qualifications
- 1+ years of experience in social work, psychology, human services setting or related field
- A background in working with nonprofit organizations, social services organizations, or community-based programs
- Familiarity with Bay Area resources for low-income families
- Bilingual abilities, especially in Spanish
- Highly proficient in Microsoft Office Suite, Salesforce, and other software
- Ability to travel locally and long distance to meet with vendors and participants
- Schedule: 40 hours with evenings and/or one weekend day required (10:00am to 6:30pm or 10:30am to 7:00pm)
