Job Summary
Case Manager coordinates client care by guiding access to housing, finances, legal issues, insurance/treatment funding, and employment resources; advocates for clients, assesses needs, facilitates group sessions, maintains documentation (treatment plans, progress notes, discharge summaries), and collaborates across departments to ensure appropriate care and transitions. Requires a Bachelor of Science in social services or related field and a North Dakota LSW or LCSW license; valid driver’s license; CPR/First-Aid preferred. Willingness to work a schedule that may include evenings, on-call, and weekends as needed.
Required Qualifications
- Requires Bachelor of Science degree in social service, human service or related field of study
- North Dakota Social Work license (LSW) or Licensed Certified Social Worker (LCSW)
- Must possess valid and current driver’s license
- Prefer CPR and First-Aid certified
