Job Summary

Conduct intake with new clients within 48 hours of assignment, developing housing plans and coordinating tours to match clients with permanent affordable housing. Execute post-placement follow-ups to troubleshoot incidents, confirm payments, and assist with lease renewals or voucher reapplications. Document all interactions, activities, and status changes accurately while coordinating with shelter specialists, care teams, and external providers. Maintain data entry and progress toward housing goals, attending staff meetings and case conferences as required. Travel throughout the five boroughs for multiple visits, with sporadic evening or weekend hours possible.

Required Qualifications

  • High School Diploma or Equivalent
  • Bachelor's degree in human services field preferred
  • Minimum of two years of experience in case management, tenant organization, counseling, community outreach and/or direct advocacy for clients in social service settings
  • Experience working with homeless individuals and people living with health and/or mental health issues and substance use
  • Ability to engage with residents
  • Knowledge of applicable state and city housing and disability laws and codes
  • Knowledge and experience working with NYC voucher programs, housing subsidies and supportive housing programs
  • Computer literacy with advanced knowledge of Word, Excel, and Outlook
  • Familiarity with HOME, CARES, CAPS, Housing Connect, etc. databases
  • Willingness to travel throughout the five boroughs and make multiple visits to the community as needed
  • Sporadic evening or weekend hours may be required
  • Demonstrated commitment to quality and data-driven reporting
  • Track record of effective collaboration within a direct service organization balancing a large caseload with competing priorities
  • Strong written and verbal communication skills
  • Action-oriented, entrepreneurial, flexible, and innovative approach to interacting with and supporting clients
  • Bilingual (or multi-lingual) is preferred
