Job Summary
Support Coordinator / Case Manager provides services to adults with IDD, ASD, and physical disabilities, developing and implementing individualized care plans, coordinating and monitoring services, and ensuring professional, cost-effective outcomes. Responsibilities include completing assessments, developing Person-Centered Service Plans, coordinating with providers, maintaining Behavioral and Crisis Plans, safeguarding confidentiality, and traveling to participants as needed (50% travel). Requires collaboration with care teams, proficiency in Microsoft Office, and eligibility trainings/certifications.
Required Qualifications
- BS/BA degree in related course work
- Experience with Microsoft Office
- Maintains CPR/First Aid certification
- Valid Driver's License including personal vehicle insurance coverage
- Current vehicle registration
- State-related clearances
- Completed trainings and certifications as required by program regulations
