Job Summary
The Case Manager/Service Coordinator will provide comprehensive case management and service coordination for adults with Intellectual/Developmental and Physical Disabilities, Autism Spectrum Disorder, and related needs. Responsibilities include developing and implementing individualized care plans (ISP), brokering and coordinating services, monitoring outcomes, ensuring participant access to housing, medical, social, and vocational supports, and maintaining collaboration with other care coordinators to ensure seamless care delivery. The role requires conducting assessments, creating Person-Centered Service Plans, guiding participants in provider selection, ensuring alignment with Behavioral and Crisis Plans, and maintaining confidentiality. Travel to participants is expected (approximately 50%), and the position emphasizes coordination across physical, behavioral, and supportive services, along with required trainings and certifications.
Required Qualifications
- BS/BA degree in related course work
- Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities
- Experience with Microsoft Office, state-maintained databases
- Maintain CPR/First Aid certification and updated state related Clearances
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration
