Job Summary
Case Manager/Service Coordinator delivers comprehensive case management for adults with intellectual/developmental and physical disabilities (including Autism Spectrum Disorder) enrolled in Medicaid Waiver. Develops Person-Centered Service Plans (ISP), brokers and coordinates services across housing, medical, social, vocational, and community supports, and monitors outcomes to ensure professional, cost-effective delivery. Responsibilities include completing assessments, ensuring provider choices, aligning Behavioral and Crisis Plans with the ISP, maintaining confidentiality, attending trainings/recertifications, and collaborating with other care coordinators to ensure seamless care. Requires 50% travel to participants, a BS/BA degree, CPR/First Aid certification, and a valid driver’s license with appropriate insurance and vehicle registration.
Required Qualifications
- BS/BA degree in related course work
- Experience with Microsoft Office, state-maintained databases
- CPR/First Aid certification and updated state-related Clearances
- Valid Driver's License with insurance and vehicle registration
