Job Summary
Coordinate and manage individualized care for adults with intellectual/developmental disabilities and autism spectrum disorder through the development and implementation of Person-Centered Service Plans (ISP). Responsibilities include completing assessments, brokering and coordinating housing, medical, social, vocational and community services, ensuring confidentiality, and monitoring outcomes to ensure professional, comprehensive, and cost-effective service delivery. Travel to participants is required (approximately 50%). Requires a BS/BA degree, CPR/First Aid certification, and a valid driver’s license, with proficiency in Microsoft Office and state-maintained databases; ongoing trainings and certifications are mandated.
Required Qualifications
- BS/BA degree in related course work
- Experience with Microsoft Office
- state-maintained databases proficiency
- Maintain CPR/First Aid certification and updated state related Clearances
- Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration
