Job Summary
Conduct person-centered assessments to identify participants' needs, preferences, and life goals. Develop, implement, and regularly update Individual Support Plans (ISP) to ensure access to housing, medical care, and community engagement. Provide advocacy and choice in selecting providers while coordinating with medical professionals, behavioral specialists, and family members. Monitor and evaluate services to maintain safety and track progress toward independence. Maintain documentation, confidentiality, and compliance with all program and ethical standards. Support both individuals with disabilities and the aging population, traveling up to 50% to meet participants in their homes. Requires a Bachelor's degree in Social Work or related field, valid driver's license, and commitment to ongoing training and certifications.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Valid driver's license, auto insurance, and reliable transportation
- Commitment to maintaining required certifications and state clearances
- Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities
