Job Summary
Conduct person-centered assessments to identify participants' needs, preferences, and life goals. Develop, implement, and regularly update Individual Support Plans (ISP) to ensure access to housing, medical care, and community engagement services. Provide advocacy and choice, helping individuals with disabilities and the aging population make informed decisions about providers and care options. Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless care while monitoring service quality and participant progress. Maintain documentation, confidentiality, and compliance with all program and ethical standards. Travel up to 50% to meet participants in their homes and communities.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Valid driver's license, auto insurance, and reliable transportation
- Commitment to maintaining required certifications and state clearances
- Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities
