Job Summary
Conduct case management services for individuals with intellectual and developmental disabilities by establishing positive relationships, exploring their preferences, and securing ongoing support services. Plan, facilitate, and continuously update Person-Centered Plans that document service needs and outcomes important to each person served. Advocate on behalf of individuals, provide resource information, and educate families on self-determination principles to empower optimal independence. Research, identify, and coordinate resources and services to achieve specified outcomes, including assisting with transitions and applying for necessary programs. Monitor plan progress through on-site visits, timely contacts, and submission of recertification reports and recommendations for new priority categories.
Required Qualifications
- A bachelor's degree from an accredited education program in a human services field
- A bachelor's degree in a non-human services field, from an accredited education program, with two years of experience in a human services field
- Ability to operate a motor vehicle
- Must be able to remain in a stationary >95% of the time
- Must be able to exchange accurate information
- Must be able to lift between 10-25 pounds
- Must be able to complete and pass a 9-Panel Drug Screening within 48 hours after accepting an offer of employment
- Must be eligible for employment (not sponsored for immigration, including H-1B, TN, and other non-immigrant visas)
