Job Summary
Care Lync is seeking a Case Manager/Service Coordinator who will manage and coordinate services for adults with Intellectual Disabilities, Autism Spectrum Disorder, and Physical Disabilities. Responsibilities include completing assessments, developing and updating Person-Centered Service Plans, coordinating providers and services, ensuring Behavioral and Crisis Plans align with the ISP, and monitoring outcomes. The role requires active coordination with care teams, locating appropriate services (housing, medical, social, vocational), maintaining confidentiality, attending trainings, and traveling up to 50% to participant sites. Qualifications include a BS/BA degree, at least 3 years of relevant case-management experience, proficiency with Microsoft Office and state databases, CPR/First Aid certification, a valid driver’s license, and current auto insurance and vehicle registration. Required Qualifications
- BS/BA degree in related course work
- Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities
- Experience with Microsoft Office and state-maintained databases
- Maintaining CPR/First Aid certification and state-related Clearances
- Valid Driver's License and auto insurance coverage
- Able to travel up to 50% of the time
- Ability to develop and implement Individualized Service Plans (ISP) and Coordinated Care Plans
- Confidential handling of sensitive information
