Job Summary
Conduct person-centered assessments to identify participant needs, develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services; provide participants with choice and advocacy to help them make informed care decisions; coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care; monitor and evaluate services for safety, satisfaction, and progress toward independence; support adults with disabilities and aging population in accessing community resources; maintain documentation, confidentiality, and compliance with all program and ethical standards; participate in ongoing training and professional development to strengthen skills and expand impact.
Required Qualifications
- Bachelor's degree in social work, Psychology, Human Services, or related field
- Strong organizational, communication, and advocacy skills
- Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities
- Valid driver's license, auto insurance, and reliable transportation
- Commitment to maintaining required certifications and state clearances
