Job Summary
Conduct person-centered assessments to identify participants' needs, preferences, and life goals. Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services for housing, medical care, and community engagement. Provide advocacy and choice, helping individuals with disabilities and the aging population make informed decisions about providers and care options. Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless care. Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence. Maintain documentation, confidentiality, and compliance with all program and ethical standards. Participate in ongoing training and professional development. Requires a Bachelor's degree in Social Work or related field, valid driver's license, and up to 50% travel.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Valid driver's license, auto insurance, and reliable transportation
- Commitment to maintaining required certifications and state clearances
- Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities
