Job Summary
Conduct person-centered assessments to identify participants' needs, preferences, and life goals. Develop, implement, and regularly update Individual Support Plans to ensure access to housing, medical care, and community engagement. Provide advocacy and coordinate with medical professionals, specialists, and family members to deliver seamless care. Monitor service delivery for safety, satisfaction, and progress toward independence while maintaining strict documentation and compliance. Support both individuals with disabilities and the aging population in accessing required resources. Requires a Bachelor's degree, valid driver's license, and up to 50% travel to meet participants in their homes.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or related field
- Valid driver's license, auto insurance, and reliable transportation
- Commitment to maintaining required certifications and state clearances
- Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities
