Job Summary

Case Manager/Service Coordination role managing an active caseload and coordinating waiver-based services for individuals with disabilities and/or seniors. Responsibilities include reviewing/updating/submitting Service Plans to RRDS, conducting monthly face-to-face meetings with participants with at least one in-home visit per quarter, ensuring PRI/SCREEN is completed annually or as needs change, developing and monitoring all services in the service plan, maintaining waiver transportation records, maintaining participant records per waiver requirements, staying informed about all approved waiver service providers in the region, and organizing/facilitating team meetings. Travel within the Capital District is required; on-site, full-time position with benefits including medical, dental, vision, life, disability insurance, telehealth, PTO, and holidays.

Required Qualifications
