Job Summary
The Case Manager supports client wellness and autonomy through advocacy, assessment, communication, and service facilitation, managing a caseload of approximately 25 families. Key responsibilities include linking participants to resources, assisting with public assistance systems, ensuring participant care is client-centered, conducting unit inspections, maintaining audit-ready records, and collaborating with external agencies. Qualifications include a Bachelor’s Degree preferred, with a minimum of 2 years in human services and the ability to engage with diverse cultures and issues related to social justice.
Required Qualifications
- Minimum of 2 years in human services
- Working experience with families and knowledge of issues related to homelessness, HIV, substance abuse, mental illness, and entitlements.
- Crisis intervention skills.
- Familiarity with Health Insurance Portability and Accountability Act (HIPAA).
Desired Qualifications
- Bilingual preferred
- Knowledge of CARES system a plus
